 Nurse Nicole’s Care Management is led by Dr. Nicole Michelle Brown, DNP, RN, a highly experienced nurse leader with over two decades of diverse clinical, academic, and care coordination experience. Dr. Brown’s extensive background includes roles as a Clinical Case Manager with Medicaid, emergency department nurse, charge nurse, nursing instructor, nursing home nurse and professor of nursing. This broad experience across acute care, chronic disease management, education, and population health has provided her with a deep understanding of patient needs across the continuum of care.

Her work as a Nurse Case Manager has been central to coordinating complex care, supporting patients with chronic conditions, reducing hospital utilization, and improving health outcomes through proactive, patient-centered strategies. Combined with her doctoral preparation and strong foundation in clinical practice and nursing education, Dr. Brown brings a comprehensive, evidence-based approach to care management.
